We start with the individual and build personalised, flexible support around them from our connected Care, Education, and Health services. So, instead of placing people on predetermined pathways, we connect the right expertise and the right service around each individual, meeting them where they are. Sometimes this means support from one service. Sometimes it means several working together. What matters is that support is centred on the individual. In doing so, we create the conditions for re-enablement and independence.
